If you are at the inspection, you will be asked to sign a consulting agreement.  If you cannot attend the inspection you should do one of the following:

1. Download the Professional Engineering Consulting Agreement using the link below, review it, sign it and fax the signed copy to 1-866-271-4805.  This is a toll-free number.  
   

2. Download the Professional Engineering Consulting Agreement, review it, sign it, scan the signed copy and E-mail the signed agreement to grayspe@Gmail.com 
   

3. If you do  not have access to a fax machine or a scanner, you can download the Professional Engineering Consulting Agreement, review it, sign it and mail it to R. Michael Gray, PE at 25414 Vinechase Drive in Porter, Texas 77365.  Then, and this is very important, send me an E-mail at grayspe@Gmail.com stating the you have reviewed the consulting agreement and accept the terms and conditions. 

We will issue only a draft report if we do not have a signed agreement.  Once I receive the faxed agreement (option number 1), the scanned agreement (option number 2) or the mail/E-mail (option number 3), I will E-mail a final copy of the report.
